For me it's happned to me quite a few times. What i aways say it "mmhhhh pls remind me your name i'm so bad in names............" pls remind me your name that's it.

just as your name suggests, makosa imefanyika, let them do the intro. actually after she introduces herself, and the other guy does so, then immediately call her by her name and say something more about her, like where she works or where you two met.

but for a dude of mine, the lady turned out be more clever. she noticed the dude doesn't remember her name, and the jamma tried to pull that trick. so the mama introduces herself as Mary, kumbe she's Grace, and the dude happily picks up the conversation and refers to her as Mary. guess what happened nxt!!
